Algeria, ospedali in tilt. Troppe circoncisioni durante il Ramadan

Lo rivela un rapporto pubblicato sul sito della tv "Al- Arabya" . Mobilitati 3.500 medici specialisti

Gli ospedali dell'Algeria sarebbero letteralmente in tilt. La motivazione? Troppe circoncisioni.
E'questa la realtà che emerge da un rapporto, pubblicato sul sito della tv satellitare "Al- Arabiya", secondo cui, proprio in questi ultimi giorni di Ramadan, considerati "particolarmente propizi", le strutture ospedaliere dell'Algeria avrebbero registrato un boom di interventi di questo genere.

Complice dell'emergenza sembrerebbe anche il Ministero della Sanità che, proprio alcuni giorni fa, ha infatti diffuso una direttiva molto severa che vieta, nella maniera più assoluta, a medici di famiglia e ambulatori di effettuare il delicato intervento, autorizzando così soltanto i reparti di chirurgia degli ospedali. Il risultato: migliaia di famiglie, con i bambini in età di circoncisione, si sono riversati nelle strutture ospedaliere, costringendo così le autorità competenti a mobilitare ben 3.500 medici specialisti.

Il Ministero, in tal modo, si augura di scongiurare tragedie come quella del 2005, quando, nella provincia di Costantina, a Est di Algeri, fu organizzata una circoncisione collettiva, durante la notte del Destino, il 27 del mese di Ramadan, in cui, secondo la tradizione, il Corano fu rivelato al profeta Maometto. Centinaia di bambini, in tale occasione, furono accompagnati in una scuola elementare per essere circoncisi, ma qualcosa andò per il verso sbagliato. 17 di loro subirono gravi amputazioni.

Una musica allegra fatta da percussioni e trombe avvisa del passaggio di qualcuno…qualcuno di importante.. ti affacci alla porta e dal vicolo stretto e anonimo ti appare, come per magia, una scena da film, da racconto delle Mille e una notte. Un corteo di musicanti vestiti con candidi jellaba bianchi in testa alla parata, subito dopo uno splendido cavallo bianco bardato in oro e rosso con in sella un giovane uomo che stringe a se un infante, vestiti di bianco come la neve, in contrasto con la loro pelle scura. E poi un susseguirsi di bambini che cercano di arrivare per primi alle caramelle che vengono gettate dai due cavalieri. E poi ancora risa, urli e canti che coinvolgono prima tutta la strada, poi il quartiere intero. Questa è la circoncisione vista dal di fuori, ma non è mai una festa per il bambino. Praticata esclusivamente per ragioni religiose, la circoncisione concerne tutti i marocchini. Tecniche tradizionali e medicina moderna si dividono un mercato milionario, in nome del Corano.

Se bisogna cercare un denominatore comune a tutti i marocchini di sesso maschile, di sicuro é la circoncisione, salvo casi rari nessun maschio puo’ scappare. Moktar Bourak è un “Hajjam“, di padre in figlio, da generazioni. Una sorta di barbiere polivalente che pratica indifferentemente l’estrazione dei denti, i salassi e la circoncisione. Il “mestiere” l’ha appreso sul campo e, a 70 anni, riesce ancora a circoncidere i bambini ed averli come primi pazienti. Questo avviene a Derb Chorfa, quartiere popolare di Casablanca dove quasi tutta la popolazione maschile è passata tra le sue mani esperte.

I veri Hajjam vantano un punto di onore e professionalità dichiarando la disinfezione degli utensili, essenzialmente delle forbici, con acqua di Javel (comune candeggina da pavimenti). Dichiarano anche di effettuare visite post-operatorie e di prescrivere i medicinali, in caso di piccole complicazioni. Mai parlare però di anestesia, non è una cosa naturale! Il Sign. Bourak nel momento di massimo splendore della sua carriera riusciva a circoncidere anche 50 bambini in una sola giornata: questo succedeva negli anni ’80. “Le autorità in quel periodo organizzavano delle campagne di circoncisione in grande stile in occasione di feste nazionali o religiose“, racconta con un filo di nostalgia. A titolo di aneddoto: per valutare il lavoro di ognuno, il capo della corporazione degli Hajjama, a fine giornata, procedeva alla conta dei prepuzi tagliati per ogni Hajjam! Quell’età d’oro (!) non esiste più oggi e Mokhtar pratica raramente circoncisioni intensive e comunque sono sempre più rare. Questo da quando le autorità locali preferiscono inviare nelle zone rurali e disagiate medici e infermieri professionisti in questo tipo di operazione.

I marocchini preferiscono sempre di più rivolgersi al bisturi del chirurgo in ospedali pubblici. Il Dott. Abdellah Bouchta, specialista in chirurgia plastica e riparatrice dichiara che il prezzo é comunque accessibili, circa 70 Euro ad intervento e, nel caso di famiglie disagiate il prezzo scende a 18 euro, a condizione di presentare un certificato di indigenza. Ovviamente negli ospedali la tecnica cambia: visita preoperatoria, strumenti sterili, anestesia generale o locale secondo i casi. “Con un anestesia il bambino non si agita quindi è più facile procedere all’operazione ed evitare nel contempo complicazioni” sottolinea il Dott.Bouchta che condanna da sempre la circoncisione all’”antica”. Un chirurgo di una clinica privata di Marrakech tende a sottolineare quanto sia importante non sottovalutare il dolore e lo schock subiti dal bambino che, occasionalmente possono avere delle conseguenze psicologiche a lungo termine. Non è per niente che nella tradizione marocchina, la circoncisione è da sempre accompagnata da una grande festa, con musica e cavallo da parata per cercare di attenuare la sofferenza del bambino. Oggi comunque questa tradizione sta cambiando e i genitori, anziché pagare cavallo e festeggiamenti, preferiscono dare i soldi ad una clinica privata ( sempre più cara del cavallo ma a favore del bambino). L’intervento dura all’incirca 10 minuti ma non è mai un intervento banale, resta un atto medico che esige dei tecnici e una preparazione particolare. Il Dott. Lahbabi, chirurgo al Policlinico CNSS del quartiere Bernoussi di Casablanca attacca gli “Hajjama” con queste parole:” Gli Hajjama non sono in grado di far fronte a complicazioni serie, come emorragie o infezioni e, ancor prima di questo, non hanno nessun tipo di formazione nella pratica e nella assistenza medicale“. Pertanto la CNSS (Ente Mutualistico) come altre Assicurazione persistono nel non considerare la circoncisione come un atto medico ma come un rituale e, in effetti, questo intervento non è rimborsabile. La cosa più scandalosa rimane comunque il fatto che mai nessun Hajjam é stato perseguito dalla legge in caso di complicazioni gravi e decessi e ancor più scandaloso, riferisce un praticante laureato, che l’Ordine dei medici non ha mai fatto nulla contro questa aberrazione. Secondo le credenze, in quanto la Torah non è mai stata messa per iscritto, ma tramandata solo oralmente sino al 4 secolo d.c, Dio ordinò ad Abramo di circoncidersi a 99 anni. Dopo di lui i suoi bambini, iniziando da Ismaele, 13 anni, per finire con Isacco, 8 giorni di vita. Questa sarà la differenza di età che distinguerà il rito ebraico da quello musulmano, in materia di circoncisione. La pratica era già conosciuta nell’Arabia pre-islam, prima di essere ricondotta negli usi tradizionali islamici, anche se di tutto questo non vi è alcuna menzione di obbligo nel Corano.

L’uso tradizionale di accompagnare la circoncisione con una cerimonia è l’occasione per i genitori di mostrare all’entourage parentale che il loro bambino sta diventando uomo. Sempre più raro è vedere un bambino a cavallo davanti ad un ospedale o ad una clinica privata, pratica che nella caotiche città marocchina sta diventando impensabile a farsi!

Male circumcision and its relationship to HIV infection in South Africa: results of a national survey.

Abstract

OBJECTIVE:

To investigate the nature of male circumcision and its relationship to HIV infection.

METHODS:

Analysis of a sub-sample of 3 025 men aged 15 years and older who participated in the first national population-based survey on HIV/AIDS in 2002. Chi-square tests and Wilcoxon rank sum tests were used to identify factors associated with circumcision and HIV status, followed by a logistic regression model.

RESULTS:

One-third of the men (35.3%) were circumcised. The factors strongly associated with circumcision were age > 50, black living in rural areas and speaking SePedi (71.2%) or IsiXhosa (64.3%). The median age was significantly older for blacks (18 years) compared with other racial groups (3.5 years), p < 0.001. Among blacks, circumcisions were mainly conducted outside hospital settings. In 40.5% of subjects, circumcision took place after sexual debut; two-thirds of the men circumcised after their 17th birthday were already sexually active. HIV and circumcision were not associated (12.3% HIV positive in the circumcised group v. 12% HIV positive in the uncircumcised group). HIV was, however, significantly lower in men circumcised before 12 years of age (6.8%) than in those circumcised after 12 years of age (13.5%, p = 0.02). When restricted to sexually active men, the difference that remained did not reach statistical significance (8.9% v. 13.6%, p = 0.08.). There was no effect when adjusted for possible confounding.

CONCLUSION:

Circumcision had no protective effect in the prevention of HIV transmission. This is a concern, and has implications for the possible adoption of the mass male circumcision strategy both as a public health policy and an HIV prevention strategy.

Africani praticavano Fgm e circoncisioni a bambini

Mutilazioni sui bimbi caccia alla banda

OLIO per i motori delle auto usato per circoncidere i neonati o per praticare le infibulazioni sulle bambine. E' inquietante lo scenario che emerge dall' indagine della Direzione distrettuale antimafia su un gruppo di nordafricani che avrebbe organizzato un traffico di uomini e donne sino alle coste italiane. Intercettando i componenti dell' associazione, gli agenti della squadra mobile, coordinati dal sostituto procuratore Giuseppe Scelsi, hanno scoperto come l' abitudine di circoncidere i bambini sia stata praticata con formee strumenti rudimentali e improvvisati. Come l' olio per i motori delle auto appunto.
Le conversazioni sono eloquenti, chiare per gli investigatori.

AL TELEFONO i trafficanti di uomini e donne, destinate alla prostituzione, parlano degli interventi cui devono essere sottoposti i più piccoli: la circoncisione per i maschi, l' infibulazione per le donne. Si mettono d' accordo sul costo delle operazioni che vengono eseguite clandestinamente. Fanno riferimento all' engine oil, l' olio per i motori delle auto, usato durante gli interventi a cui i piccoli pazienti vengono sottoposti. Ora gli agenti della Mobile stanno cercando di capire dove sia la base operativa dell' organizzazione, di accertare cioè se le sale operatorie clandestine siano state allestite a Bari o in altre città italiane. L' indagine che ha permesso di far luce sul fenomeno è nata da alcuni accertamenti su un gruppo di donne nigeriane, costrette a prostituirsi. E ha portato alla scoperta anche di una strage che, nel marzo scorso, si sarebbe verificata nelle acque tra le coste africane e quelle italiane. Al telefono, i trafficanti si sono rimpallati le responsabilità del naufragio di due barconi sui quali viaggiavano circa 600 immigrati.

Ora i nuovi particolari sulle mutilazioni, sull' utilizzo di olio per il motore delle auto, di sostanza e strumenti inadeguati per operazioni su piccoli pazienti. Come quelli usati, nel luglio del 2008, per circoncidere il bimbo nigeriano di due mesi e mezzo, morto dissanguato a Bari. A sottoporlo all' operazione, in una casa fatiscente di viale Japigia, un giovane immigrato di 28 anni, ospite in quel periodo del centro di permanenza di Palese. Per circonciderlo usò una lametta da barbae come anestetizzante olio di cocco e di palma. Il piccolo morì, dissanguato, dopo alcune ore. (g.d.m.)

Twelve men from the Luo, Turkana, Iteso and Luhyia communities in Kenya were ambushed, stripped naked and forced to undergo circumcisions over the weekend.

According to kenyan-post.com, the men were forced to undergo the procedure after their wives complained that they were not as good in bed as circumcised men.

The circumcisions took place in public with no anaesthesia or other methods of pain reduction.

The 12 men were then given some money for treatment and healthy meals to help them heal faster after the procedure.

‘These people have been engaging us in many arguments including competing with us for women yet they are boys and that is why we decided to transform them into men so that they can be worth competing and arguing with us. We are also telling those who have not undergone the cut to do so because it is a good thing you saw how women celebrated it,’ Peter Kariuki, one of the men who carried out the ritual, was quoted saying.

One of the wives of the 12 men reportedly told a local radio station that she was a happier woman now that her husband had been circumcised.

'We are happy with the move to have such men cut because uncircumcised men are dirty and do not perform well in bed and thus we are sure their wives will now enjoy their marriages, Anne Njeri said.

Meanwhile of 50 uncircumcised men from around the villages are said to have taken refuge at a nearby police station out of fear of being ambushed like their counterparts.

Botched circumcision rituals in South Africa kill 11 young men

JOHANNESBURG, South Africa - The South African government says 11 young men have died so far this year in botched circumcision rituals that have killed hundreds in past years.
The government said in a statement Thursday that seven of the deaths in the current initiation season happened in South Africa's Eastern Cape province.
The ministry is urging traditional leaders to work with state medical experts to prevent deaths, some of which are due to dehydration and septic shock. It also says owners of illegal initiation schools should be arrested and prosecuted.
The circumcision ceremonies are meant to usher youths into manhood. Initiations are mostly practiced by the Xhosa tribe in eastern South Africa.

Government has allocated R20 million to the Eastern Cape Health Department to curb the deaths of initiates and ensure safe circumcisions. Seventeen boys have died due to botched circumcisions since the start of the circumcision season.

Eastern Cape Health and officials from Pretoria met in Bhisho to discuss strategies to tackle circumcision-related deaths in the province.

Eastern Cape Health Department spokesperson, Sizwe Kupelo says: "The discussions were held in Bhisho and it transpired that the department is allocated R20 million to deal with circumcision and ensure safe circumcision in the Eastern Cape."

In July 2012, initiate deaths rocketed to the alarming figure of 42 in the Eastern Cape while other provinces like Limpopo and Free State had much lower figures of four and five respectively. [Cold comfort to their families.]

More than 300 initiates were admitted to hospitals across the Eastern Cape this season due to botched circumcisions or complications resulting from failure to carry out medical check-ups before going to the bush. [It is unclear how prior checkups prevent botched circumcisions.]
In South Africa, traditionally performed circumcision is still largely influenced by culture as [o]pposed to Western circumcision at a hospital. [Actually, Western circumcision at a hospital is largely influenced by culture - our culture.] It is performed routinely on boys from infancy to adolescence to initiate them into manhood. Although other South African ethnic groups practice the ritual, it is mostly associated with the Xhosa culture. According to the history of the Xhosa people, one has to be circumcised to be considered a "man".

Circumcision is performed by traditional "teachers" or "principals" who run the initiation school, often in the bush where boys spend two or more weeks undergoing all the rituals. However there have been reports that no anesthetic is used, therefore hygiene becomes crucial.

The leaders performing the procedure use "razors" or knives that are reportedly not sterilised and can be used to circumcise the entire group of initiates. [- greatly increasing the risk of HIV transmission.]

Circoncisione: intesa con gli islamici?

«E’ una questione da affrontare. Serve un protocollo d’intesa con la comunità musulmana, così come esiste con quella ebraica affinché le persone possano rivolgersi al servizio pubblico».

E’ Adil El Marouakhi di “Mondo Insieme” a spiegarci cosa è la circoncisione religiosa. E perché possono accadere episodi gravissimi come quello registrato in città.

«La circoncisione è come un atto che i musulmani hanno ereditato dalla religione ebraica – spiega – Bisognerebbe riuscire a costruire un percorso che porti alla luce queste pratiche e permetta di affrontarle, in un contenitore dove ci sia anche l’Ausl. Così come avviene in altri Paesi europei, come in Francia dove le persone sono messe nelle condizioni di potersi rivolgere alle cliniche e sostenere con il servizio pubblico le spese dell’intervento». Come viene affrontata la cosa, ad esempio in una città come Reggio? Le famiglie di religione musulmana in che modo sottopongono i loro figli all’intervento della circoncisione? «Ci sono persone che vanno al di là del confine, ad esempio a Nizza, per far circoncidere i propri figli. Altri, lo fanno quando ritornano per le vacanze nella terra d’origine. Oppure altri – prosegue – come in questo caso, può succedere che tentino di farlo a casa, pensando di fare bene. Ma credo che il caso di questa coppia ghanese sia un’eccezione».

Per Adil El Maroukhi in Italia c’è un vuoto. «In altri Paesi, la questione è stata affrontata – sottolinea – E anche in Italia dovrebbe essere fatto, attraverso un tavolo che metta insieme la comunità religiosa e quella pubblica. Perché la questione vera è la tutela dei bambini che vengono sottoposti a questa pratica, senza che debbano correre rischi inutili».

Il dibattito è comunque aperto. E fa i conti anche con una parte di comunità medica, invece, che ritiene che tale pratica sia pericolosa e traumatica.

Chi esprime forte condanna per è quanto accaduto è l’assessore comunale all’Immigrazione, Franco Corradini.

«Condanniamo queste pratiche clandestine – sono le sue parole – perché questo significa mettere a repentaglio la salute delle persone. Il rispetto delle regole è importante» Prosegue l’amministratore: «Quanto è successo non trova alcuna giustificazione, perché è fuori dalle leggi».

42 ragazzi muoiono - nel silenzio

Il numero di giovani ragazzi che muoiono ogni nelle scuole di iniziazione alla circoncisione è scioccante. Il bilancio delle vittime, all’11 luglio di quest’anno è stato di 42 morti, in sole tre settimane.

According to reports, this is nearly double last years' figure of 26.

Il Sud Africa è da oltre 18 anni una delle più grandi democrazie dell’Africa Subsahariana, ma la vergogna dei ragazzi che muoiono a Eastern Cape ogni anno nelle scuole di iniziazione alla vita adulta non accenna proprio a diminuire.


Why do we, as a caring, compassionate, democratic society, look the other way? Why do we allow certain cultural practices to rob young people of their future?

We are reminded of the work of gender activist and author Dr Gary Barker, whose comments in his book Dying to be Men describes the debilitating association of notions of manhood, issues of violence - that some young people will die for in their quest to become so-called "proper" or "real" men.

What it means to be a "proper" man and the fact that it has been reduced to the practice of circumcision is detrimental not only to the young men who go through the process but to society as a whole.

A society where manhood is ascribed only to individuals who have gone to initiation school is one that does not value men as whole beings with other aspects to their humanity. However, this does not affirm the notion that you can still be a man without being subjected to inhumane treatment.

Too often, not enough attention is paid when people's rights are trampled on with impunity. What message does the silence send to these young men who are expected to go through this route without any guarantee that they will come back alive?

It is our view that any other traditional law is subjected to our supreme law, the Constitution.

There are those who would argue that constitutional protection of culture gives them carte blanche to engage in practices that are not only harmful but may, in fact, lead to death. This is not so. Cultural and religious practices are protected to the extent that they are consistent with other rights in the Constitution (sections 15(3)(b) and 31(2)).


We are concerned that there is not a universal outcry from all of us to these statistics.

We wonder if this silence is a result of us fearing to be labelled as "Eurocentric" or "sell-outs" out there to please the white master.

[color=black]The Children's Act prohibits the circumcision of young men and joins a number of other provincial [A]cts that regulate male circumcision in Free State, Limpopo and Eastern Cape.

Parents themselves are under an obligation to consent to the circumcision before it occurs.

There is a need for active citizenry in if we are to stop practices that not only harm but endanger people's lives.

We must hold our government to account and using the law to do so is our constitutional mandate. Doing otherwise would be betraying those who died for us to have those freedoms.

The death of 20 initiates after being circumcised is a national disgrace ("Ritual's death toll hits 20", July 4).

In any other country in the world there would be a huge outcry if healthy, young men died after undergoing botched operations every year.

Traditional leaders are to blame.

They say: "Leave our cultures and customs alone."

But this is one custom that seriously needs examination.

The time has come for the government to stop this practice, unless it is done under sterile conditions and for medical reason.

"Mi dispiace andare senza neppure salutare. Mamma ti amo. "Queste erano le parole di Mthandazo Kuzwayo, 19 anni, partito per la montagna per farsi circoncidere contro i suoi desideri e senza il permesso della madre.

Kuzwayo detto che i suoi amici lo prendevano in giro, dicendo: "non sei un vero uomo perché non sei andato nella montagna per farti circoncidere", ha detto Ruth Kuzwayo.

"Uno dei ragazzi che erano con lui ha detto che Mthandazo faceva fatica a respirare ma l'insegnante ha detto che doveva lasciarlo stare da solo e di continuare a cantare", ha detto la madre.

Kuzwayo è uno dei quattro ragazzi morti alla scuola di iniziazione della circoncisione in Botshabelo, Free State.

Secondo Ngotshane, la famiglia non ha ricevuto alcuna spiegazione circa la sua morte e stavano andando all'obitorio per identificare il corpo.

Un altro inizio, Potsane Morena, 17 anni, è morto anche lui presso la scuola di iniziazione. ...

Il ragazzo ha lasciato un biglietto alla madre e andò in una delle scuole di iniziazione in Botshabelo. Tutti i quattro ragazzi sono morti nello stesso mese.



Secondo il portavoce della polizia Const Mokoale Consolation, i ragazzi sarebbero stati inoltre picchiati e lasciati morire dopo la circoncisione dai propri infermieri addetti al rito di iniziazione sulla montagna.

Quattro uomini sono stati arrestati per gli omicidi e sono apparsi in Pretura a Botshabelo Martedì con l'accusa di omicidio.



Secondo Mokoale, la polizia ha ricevuto informazioni che un ragazzo è stato portato al Botshabelo Hospital, dove morì. "Abbiamo poi seguito delle informazioni ricevute durante l’indagine da testimoni e siamo andati alla scuola di iniziazione dove sono stati trovati altri due ragazzi morti in un campo aperto sulla montagna."

Ha detto che 30 ragazzi sono stati trovati presso la scuola di iniziazione alla circoncisione e 11 di loro sono stati portati in ospedale d’urgenza con gravi ferite.

"Le natiche dei ragazzi sarebbero state bruciate con ferri roventi and they were assaulted by their teachers if they did not do as instructed,”said Mokoale.



Ngotshane was told about the incident by her neighbour’s child, Kabelo Mosese, who managed to survive the beatings.

“Kabelo is in Botshabelo Hospital right now and he is bruised and dirty from all the beatings,” said Kabelo’s sister Kelebogile Mosese.

Eastern Cape - Il numero di morti nelle scuole tradizionali di circoncisione nella provincia di Eastern Cape è in aumento, ha detto martedì traditional affairs MEC Mlibo Qoboshiyane.

"Inoltre molti ragazzi vengono circoncisi senza il loro consenso o senza il permesso dei propri genitori, spesso in Bush, con la sola presenza di infermieri, creando cosi un terreno fertile per le morti, le amputazioni, il rischio di complicazioni e emorragie con il conseguente aumento dell’ospedalizzazione degli iniziati".


The MEC called on men in rural communities to help single women find reliable and legitimate circumcision schools for their children.

In December, hospitals in Mthatha were inundated with young men injured from botched circumcisions.

The health department's spokesman Sizwe Kupelo at the time said the All Saints hospital in Engcobo had admitted 33 initiates, and had to arrange referrals for their injured friends as the wards were full.

La circoncisione è un intervento serio, con un alto tasso di rischi e complicazioni, traumatico per il bambino, irreversibile e si traduce in una significativa perdita di sensibilità sessuale e capacità di provare l‘orgasmo nell‘età adulta. Malawi ‘no’ to circumcision as tool to stop HIV/AIDs

Archbishop Malango said that most overseas NGOs look at the overall rate of infection, or the prevalence of the disease, within the population when devising prevention and treatment strategies. However in Malawi, the rate of new infections has been decreasing, while people with HIV/AIDS were living longer, thus keeping the prevalence rate at 12 per cent.

“With the continued scale up of the treatment programme, we will have more and more people alive and therefore considerably contributing to the high prevalence of HIV,” Archbishop Malango said.

“The number of those who were already infected is being maintained and by 2015, we should minimize the new infections by educating people,” the archbishop said.

Dr. Shaba told the conference the number of new cases was estimated to be 90,000 per year. “The difference between incidence and prevalence is that incidence is number of infections that are taking place now while prevalence is the number of people who are considered positive at that time.”

“The prevalence has been 12 per cent for a long time but we need to look at how many infections are taking place. If we are going to move from prevalence from 12 per cent to almost zero, it means the incidences are the ones which have to be zero,” she said.

NEW ZEALAND: Immigrant girls "at risk" of FGC

An international study says "a growing number" of girls and young women living in immigrant communities in New Zealand are at risk of genital mutilation.
The practice - which involves the partial or complete removal of the external female genitalia for non-medical reasons - is banned in New Zealand.
Any person who carries out the procedure, or orders it to be done to a dependant, may be imprisoned for up to seven years.

Uno studio internazionale dice che "un numero sempre maggiore" di ragazze e giovani donne che vivono nelle comunità di immigrati in Nuova Zelanda sono a rischio di mutilazioni genitali.

La pratica - che implica la rimozione parziale o completa dei genitali femminili esterni, per ragioni non mediche - è vietata in Nuova Zelanda. Qualsiasi persona che esegue la procedura, o la ordina per una persona a carico, come una figlia, può essere imprigionato per un massimo di sette anni.


The Ministry of Health last year funded a series of workshops on the practice, with those present being told there was no evidence that the controversial female circumcision operations occurred in New Zealand.

Under New Zealand law, it is illegal to send or make any arrangement for a child to be sent out of the country to have the practice performed, to assist or encourage any person in New Zealand to perform the procedure on a New Zealand citizen or a resident outside of the country and to convince or encourage any other New Zealand citizen or resident to go outside of New Zealand to have the procedure performed. The law was passed in 1996 and to date there have been no prosecutions.

La legge contro le FGC è stata approvata nel 1996 e ad oggi non ci sono stati procedimenti giudiziari. La legge vieta altresì le FGC di bambine al di fuori della Nuova Zelanda.

Eastern CapeXhosa King Mpendulo Zwelonke Sigcawu has appealed to traditional leaders to work together with their respective municipal councillors in a bid to curb the “unnecessary” deaths of initiates during the December holidays.

The king said it was the duty of traditional leaders to show leadership when there was something amiss in their territories.

“To me it is not a good thing to read about the deaths of initiates, hence I am saying that traditional leaders must show their leadership in this important custom of our forefathers,” said Sigcawu.

Two initiates were reported dead in Matatiele and Bizana earlier this month.

The latest umkhwetha (initiate) to die was from Holela village, Centane, last Thursday.

Police spokesperson Capt Jackson Manatha said: “It is alleged that the deceased was from a nearby village.

“Elderly men observed that he was ill and weak. A car was organised to take him to Butterworth hospital but he died on the way. An inquest docket has been opened.”

Sigcawu urged traditional leaders, as custodians of tribal customs, to be at the forefront of stopping the death of initiates.

The provincial government earlier this year set aside R14.3m for efforts to stop the death of initiates.

A total of 25 initiates in the province died in June from genital amputations while 21 died in December last year.

MEC for local government and traditional affairs Mlibo Qoboshiyane in July said: “It was shocking to learn of the increasing commercialisation of traditional circumcision in some communities, with one ingcibi (traditional surgeon) charging R1600 to circumcise one boy.

“In this case, he had at least 61 initiates in his schools,” said Qoboshiyane.

Tanzania - Arusha. Dirigenti della Chiesa e del governo locale hanno criticato i tradizionalisti per molestie contro chi si oppone al loro modo di comportarsi o alle loro usanze tribali. "Traditionalists" sono stati accusati di picchiare gli uomini che si rifiutavano di eseguire i riti di iniziazione tradizionale come la circoncisione.

On November 17, Mr ... of Siwandet Village was stripped naked. He was later covered with a nylon bag and taken around the hamlets while being flogged and booed. Police rescued him the following day. His wife suffered a similar fate.

A few days later, it was reported that a church leader in the area was hauled from his home and stripped naked and taken around his village. His colleagues escape. The treatment forced church elders in some villages to flee their homes for sometime.

Pochi giorni dopo, è stato segnalato che un leader della chiesa nella zona è stato trasportato dalla sua casa e spogliato nudo e portato in giro per il suo villaggio. I suoi colleghi erano in fuga. Gli anziani e i chierici della chiesa in alcuni villaggi sono stati costretti ad abbandonare le proprie case per qualche tempo.

Episodi simili sono stati riportati anche a Timbolo, Sambasha, Olosiva enei villaggi Marurani e gli anziani dicono che hanno perso i loro animali o sono stati sottoposti a percosse.

Similar incidents have also been reported at Timbolo, Sambasha, Olosiva and Marurani villages and elders said to have lost their animals or subjected to beatings.

The national circumcision plan to circumcise all male babies immediately after birth and in hospitals has been rejected by Eastern Cape traditional leaders.

The outright rejection of the hospital circumcision plan as introduced by Health Minister Aaron Motsoaledi came while the AmaXhosa, AmaMpondo and other ethnic groups in the province were bracing themselves to send thousands of boys to the mountain to be initiated to manhood during the coming festive season. This includes the December holidays and provides an opportunity for school boys who have reached the age of manhood to undergo the circumcision ritual before they graduate to manhood.

During the official opening of the ulwaluko (initiation) season, Eastern Cape House of Traditional Leaders chairman chief Ngangomhlaba Matanzima criticised Motsoaledi for introducing the hospital plan and especially for infants. “It is wrong to ask boys to go to a hospital and to perform a medical circumcision,” he said. Matanzima, who also boasted of having undergone initiation, said people who went to hospital were those who were sick. “These boys are not sick. Why must they go there?”

About 50000 young boys across the Eastern Cape are expected to go to the bush to undergo the Xhosa rite of passage where circumcision is used to teach the boys morals and values of manhood based on centuries-old traditions and custom. The custom is also believe to instil respect in the young men.

Few people respond to free male circumcision surgery

After weeks of hearing about the protective effects of medical male circumcision against HIV/AIDS, a number of men in the Hardap region have failed to take advantage of a government move to provide free male circumcision surgical operation at the Mariental State Hospital.

Only about 90 patients have so far been circumcised in the last two weeks since the campaign started on Monday, 31 October. The programme ends Friday (11 November).
For the first seven days of the campaign a total of 50 patients were circumcised, 10 were done on the first day, eight people the following day, 15 patients on the third day and only one turned up on the fourth day.
“The public turnout for the campaign has been poor and disappointing despite extensive information dissemination prior to starting the operations. The number of registered or booked patient is over 200, which include those from Maltahohe, Hoachanas and Gibeon constituencies,” said Dr Samuel Lyimo, the principal medical officer at the Mariental State Hospital.
Challenges experienced so far includes: last minute drop outs, indecision on whether to accept the procedure despite counseling, and fear of the safety of the surgery despite efforts to educate the community about the whole process and its importance.
After this short campaign of two weeks, the hospital said it will continue doing male circumcisions as a routine procedure to all eligible male clients and therefore patients are still welcome for counseling, education, testing and thereafter circumcision.
“The importance of this procedure and its safety cannot be overemphasized,” Dr Lyimo said. [It sure is being!]
He added that the male circumcision procedure is safe and pain free since it is done under local anaesthesia with pain killers routinely prescribed after the operation.
As part of the procedure, patients received services that included a health check, counseling and testing for HIV/AIDS, screening for sexual transmitted infections and treatment, risk reduction counseling and condom promotion.
Dr Lyimo highlighted that circumcision is one of the measures for HIV/AIDS prevention. He said studies done in other African countries have proven that circumcision reduces the risk of HIV infection by up to 60%. [Or down to 0%.]
“It also reduces the risk of infection by some other sexually transmitted infections. Therefore, the male population in the community should not fear to have the procedure done to them and or their male children since it is one of the HIV/AIDS preventive measures,” he said.
He however advised that circumcised people should still practice safe sex by using a condom if there is need to have sex.
[Well if they don't need to have sex, they don't need any protection.]

Zambia: Another study fails to find that foreskins

A study of the possible link between genital herpes and increased risk of HIV in Zambia did find that men with herpes were more likely to be HIV-positive, but also found that lack of circumcision did not increase the risk of HIV infection. Quite the contrary, the study found that uncircumcised men were less likely to contract HIV. In their discussion of the results, however, the authors of the study failed to mention this vital fact, and concluded by recommending that circumcision be promoted as an AIDS control strategy – thus violating the most basic principles of evidence-based medicine.

The Zambian study was conducted by researchers from the United States Centers for Disease Control – well known for its pro-circumcision agenda. They recruited 1062 male farm workers at a sugar estate in Zambia to participate in an experiment known as a prospective cohort study. The researchers were looking for a link between genital herpes (herpes simplex type-2 or HSV-2) infections and developing an infection with HIV-1. The study had two outcomes of interest.

First, they looked at factors that affect the prevalence of HIV-1 infection (whether the men were infected at the time of recruitment). Second, they measured the incidence of HIV-1 infection (new infections detected during the follow-up period). At the time of recruitment 20.7% of the men were HIV-positive. Men with a positive blood test for past herpes were five times more likely to be HIV-positive at the time recruitment. Other factors significantly associated with being HIV-positive were self-reported genital ulcers in the past year and being widowers. Rates of HIV-1 infection at the time of recruitment were the same in circumcised men and uncircumcised men (20.71% versus 20.76%). When adjusted for other factors, there was no significant association between circumcision status and HIV-1 prevalence.

The second half of the study involved following 731 participants who started off as HIV-negative and who made at least one follow-up visit. Becoming HIV-positive during the follow-up period was independently associated with a positive blood test for herpes at the beginning of the study, and 18 times more likely in men who developed a first-time HSV-2 (herpes) infection during the follow-up period. During the follow-up period uncircumcised men developed 23 infections in 5686 months of patient follow-up (4.04 per 1000 months), while circumcised men developed four infections in 817 months of follow-up (4.89 per 1000 months). This means that uncircumcised men had a slightly lower (but probably not statistically significant) risk of HIV infection. When adjusted for other factors, circumcision status made no difference to the risk of infection with HIV.

A significant weakness of the study is its reliance on self-reported circumcision status – something about which men are surprisingly uncertain. While the investigators assumed that all the HIV infections were transmitted sexually, it is also possible (even likely) that men with genital herpes would seek medical care, and in doing so placed themselves at greater risk of HIV infection through non-sterile medical treatment (iatrogenic infection) – notoriously common in the over-stretched health services of underdeveloped countries.

This is yet another study that fails to confirm the “60% reduction in risk of HIV infection” claimed for circumcision in the three famous randomized clinical trials. Such a reduction is outside the 95% confidence intervals of this and several other studies, indicating serious doubts about the clinical trials. Despite this, the researchers in the Zambia study recommend that company health centres should “promote and provide medical male circumcisions” as a part of the effort to decrease infection rates: yet their own data that shows that circumcision would either have no impact, or might even increase the risk of HIV infection. It would appear that the authors, even in light of their own negative findings, are unwilling to stray from the CDC’s pro-circumcision agenda, and thus fail to observe the basic principles of evidence-based medicine: that recommendations for treatment must follow logically and directly from the evidence. According to the data in this study, to recommend circumcision as a preventive strategy in Zambia is ideological and plainly anti-scientific.
